• Payment — to bill and obtain payment from you, an insurer or a third party.
• Health Care Operations — to run our practice, improve care and for quality assessment.

• The right to inspect and request a copy of your health records
• The right to request a correction to your records
• The right to request restrictions on certain uses and disclosures

Complaints

If you believe your privacy rights have been violated, you may file a complaint with Livingston Chiropractic & Rehab at (973) 597-9766 or with the U.S. Department of Health and Human Services. You will not be penalized for filing a complaint.
